The Reference Code is a 1-15 digit number located at the top right corner of the Notice of Vehicle Insurance Status' header.
The Response Required By is located below the Notice of Vehicle Insurance Status' header at the right-hand side and indicates the deadline for submitting a response to the notice.
Use the calendar on this page to select the date matching the Response Required By indicated in your notice.
The Vehicle Identification Number (VIN) of the vehicle referenced in the notice can be found in the Vehicle Information table.
In order to submit this form, you need to provide the last 6 characters of the VIN. If the VIN is shorter than 6 characters, then enter the full VIN.
